CTS
Registration Procedures
| 1.
|
Company should
be a valid Investment Incentive Group (IIG)
Claimant of DOF- CENTER. |
| 2. |
Company
should submit to the DOF-CENTER a sealed
envelope containing the following: |
| |
a.
|
Duly filled
out CTS Agreement/Enrollment form along
with the complete signatures of the authorized
signatories of the company |
| |
b.
|
Two (2) duly
filled out CTS Access forms enclosed in
separate sealed letter envelopes (CTS Representatives
must sign the sealed envelopes aside from
the forms) |
| |
c.
|
Latest Secretary's
Certificate containing the authorized signatories
of the company (notarized). |
| 3.
|
The envelope
containing all these documents should be
addressed to: |

The name of the two (2)
CTS Representatives as determined and
authorized by the enrollee/company should
be provided in the CTS Agreement/Enrollment
form. These Representatives will be given
access over the company's CTS on-line
account.
The DOF-CENTER strongly recommends that
for the company's control purposes, the
two (2) CTS Representatives should be
composed of:

a.
|
CEO/
a "high ranking official" |
|
b.
|
Company
personnel who transacts regularly
with the CENTER |
|
| 5.
|
These appointed CTS Representatives
of the enrollee/company should provide
the MIS-CENTER on the CTS Access Form
( to be sealed on a letter envelope with
the CTS representative's signature on
the envelope's opening) the following
details:

| 6.
|
The USER ID shall consist
of a maximum of eight (8) " letter" characters
and shall serve as Name ID which will
give the CTS Representative initial access
to the company's CTS account.
|
| 7.
|
The PASSWORD should consist
of a maximum of eight (8 ) alphanumeric
characters and shall serve as the final
access to the CTS.
|
| 8.
|
For change of CTS USER
ID and PASSWORD, the Representative can
effect such in the "Changes Menu" of the
on-line CTS. This will allow the CTS Representative
to change his CTS security on the internet.
|
| 9.
|
For change of CTS Representative/s,
the company should forward to the MIS-CENTER
the following:
|
a.
|
Secretary's
Certificate indicating the newly authorized
CTS Representative/s |
|
b.
|
CTS
Access Form containing the USER ID
and the Password
of the new CTS Representative/s. |
|
| 10.
|
For change in Claimant
Information, the company must notify the
CENTER immediately through the CTS Feedback
Menu, telephone or mail of any changes
of information, such as but not limited
to change of the company's address, telephone
number(s) and email address.
|
| 11.
|
In logging-in, CTS access
will be denied if in case the CTS Representative
enters an erroneous PASSWORD and User
ID .
|
| 12.
|
It is agreed that the
security and confidentiality of the USER
ID and PASSWORD shall be the responsibility
of the company.
|
| 13.
|
The CENTER reserves
the right to amend, revise or supplement,
from time to time, any of the terms and
conditions governing CTS without notice
to the claimant. All such changes, revisions
or amendments shall, however, be posted
for a period of at least Fifiteen (15)
consecutive working days in a conspicuous
place in the CENTER's premises and/or
in the CTS / CENTER's Web Page, and shall
become effective upon the expiration of
such 15-day period.

are readily available at the MIS-CENTER.
Company can also download all forms
on-line via the CTS Registration Menu.
