What
is Claim Tracking System (CTS)? |
|
The Claim Tracking
Systems (CTS) is an internet-based
facility service offered by the
DOF-Center to its claimants. It
is designed to generate transparency
and governance as to the processing
of claims.
|

What
are the basic features of the CTS? |
|
The
CTS will provide the claimant pertinent
information which are as follows:
|
a.
|
summary
of all pending claims
|
|
b.
|
current
status/location of each claim
|
|
c.
|
basic
claims information such as
CIS #, application #, date
of filling, amount applied
for
|
| d. |
date
of filing and acceptant of claims
|
| e. |
deficiencies
and deadline for compliance
(if applicable) |
| f.
|
issues
affecting the claims and their
status (if applicable) |

| 3.
|
Who
are qualified to avail of this facility? |
|
The CTS on-line
facility is exclusive
for the enrolled claimants of the
CENTER. During this interim
stage, the qualified claimants are
those who are applying for tax credit
filed with the Investment Incentive
Group of DOF-Center.
|

Enrollee/Company
should submit to the MIS-CENTER
a
sealed envelope containing the following:
|
a.
|
Dully
accomplished CTS Agreement/Enrollment
Form along with the complete
signatures of the authorized
signatories of the company.
|
|
b.
|
Two
(2) duly filled out CTS Access
Forms enclosed in separate
sealed letter envelopes
|
|
c.
|
Latest
Secretary's Certificate containing
authorized signatories of
the company (notarized)
|
The
envelop containing all these documents
should be addressed to:

What
does the CTS Agreement / Enrollment
Form contain? |
|
Generally, this
contains the terms and conditions
of the CTS. Likewise, a space is
provided for your designated CTS
Representatives who will be given
access to your company's on-line
CTS transactions. And of course,
this has to be duly signed by your
company's authorized signatories.
|

How
many "CTS Representatives"
is my company allowed?
|
|
Your
company is entitled to two (2) CTS
Representatives. They will be given
access over the company's CTS on-line
account.
The DOF-CENTER strongly recommends
that for the company's control purposes,
your two (2) CTS Representatives
should be compose of:
|
a.
|
CEO/
a "high ranking official"
|
|
b.
|
Company
personnel who transacts regularly
with the CENTER
|
|

What
does the CTS Access Form contain?
|
|
The CTS Access
Form, which should always be sealed
in a letter envelope and signed
by the corresponding CTS Representative
on the letter envelope's opening,
contains the USER IS, PASSWORD and
name with signature of that CTS
Representative.
|
|
|
| 8. |
What
are the USER ID and PASSWORD for? |
|
Both are needed
for you to access your CTS. The
User ID serves as your ID and should
consist of a maximum of eight (8)
characters, all in letter. The PASSWORD,
on the other hand, will give you
final access to your CTS and should
contain a maximum of eight (8) alphanumeric
characters.

How
much do I have I to pay so that I
can enjoy the benefits of CTS? |
|
The
CTS is for free!
No application fee, no monthly dues!
|

How
long does it take for my company to
be enrolled assuming that all my documentary
requirements are complete? |
| It
will just take 24 to 48 hours before
you can access your CTS on-line. |
| 11. |
Where
can I access my CTS? |
|
Since
CTS is an internet-based facility,
you can access it anytime, anywhere.
|
| 12. |
Can
I access my CTS at the Center?
|
| Of
course. We would be very happy to
assist you with your CTS transactions. |

What
if I want to change my USER ID or
PASSWORD?
|
|
You
can change you USER ID or PASSWORD
on-line via the "Changes
Menu"
of your CTS.
|
| 14. |
What
if I want to change my CTS Representative/s?
|
|
For
change of CTS Representative/s,
you should forward to the MIS-CENTER
the following:
| a. |
Secretary's
Certificate indicating the newly
authorized personnel/s |
| b. |
CTS
Access Form containing the USER
ID and the PASSWORD of the new
CTS Representative/s. |
|

Where
can I get a copy of all the CTS forms?
|
|
The Agreements/Enrollment
an Access Forms are available on-line,
all you have to do is go to the
Registration Menu of your CTS and
download or print! The said forms
are also available at DOF-CENTER.
|
| 16. |
What
if I entered erroneously my USER ID
or PASSWORD? |
|
You
will instantly be denied access
to your CTS if you entered a wrong
USER ID or PASSWORD.
|
| 17. |
What
if I forgot my password? What if
an unauthorized personnel got hold
of it? What if my designated representative
resigns?
|
|
If
you forgot your password, notify
the Center immediately so we can
deactivate it and re-enroll you
again. Contact Mr. Nap Cajucom./
Bring Dimatulac of MIS-Center through
telephone nos. 526-6972 or 526-2293
or use your CTS-Messages Menu in
contacting Office of Special Concerns.
Remember, however, that it is agreed
that the security and confidentiality
of the USER ID or PASSWORD shall
be your company's responsibility.

access the list of deficiencies
of your particular claim, just click
"deficiencies"
as shown in the details of your
specific claim. You will automatically
access the listing to be accomplished
and the due date for compliance.
We
encourage you to access the status
of your claims diligently so that
you can effectively monitor the
required documents. So much time
will be saved if you access the
relevant information on-line rather
than having to wait for the standard
mailed letter, phone call, facsimile
or having to wait for your authorized
personnel to pick-up the deficiency
letter.

the full explanation of each along
with the corresponding recommendation.
You can also access your claims
with policy issues in the Summary
Menu.

is the Search
Menu all about?
|
|
The
Search Menu would allow you to locate
the details and status of a particular
tax credit application via CIS#,
Process, Status or age.
|

is Messages
Menu? |
|
The
Messages Menu which is being handled
by the Office of Special Concerns
would enable you to communicate
with the Center on-line. Its built
in feedback mechanism would also
permit you to air out your comments
and suggestions. Its two sub-menus
namely Inbox
and Compose have the following
functions:

- where you can get our response
/ letters
Compose
- where you can write your concerns,
queries, feedback.

Menu? |

This Menu function
allows you to change your USER Name
and Password as often as you like,
anytime and anywhere!

is Office of Special Concerns?
|
|
Office
of Special Concerns was established
by the new administration to further
its commitment to an efficient,
transparent, fair and quality service
to its constituents. It is CENTER's
"Help Desk",
compose of a committee dedicated
to bring quality and prompt service
to the claimant's concerns, queries,
suggestions and feedback regarding
the operations, procedures, policies
and projects of the CENTER.

can I be a part of the development
of CTS? |
|
You
can write your comments and suggestions
on how we can further improve our
CTS through the Messages
Menu. Rest assured, however,
that CTS will be proactive and self-evolving
in its systems enhancements. You
can access updates regarding CTS
developments from the CENTER's new
WEB SITE.
