modes of enrollment are either by
FAX or ONLINE. If you choose to
enroll online, please click the
link below. Otherwise, you may fill
up the enrollment form that was
sent to you and fax it to (02) 526-0076.

are also required to submit the original
copy of the following to the CENTER: |

a. |
Updated
secretary's certificate OR confirmation
in writing attesting to the validity
of your secretary's certificate
which is already on file with the
CENTER in case there are no changes
in your authorized representatives, |

b. |
Certified
true copy (CTC) of the General Information
Sheet(GIS) as submitted to the Securities
and Exchange Commission, and |

c. |
Letter
confirming the identity of the person
Authorized to Access and Communicate
with the CENTER through its website. |

| The
above documents are required regardless
of whether you enrolled online or
by fax. Only upon receipt of the original
copy of the same can we complete the
processing of your enrollment. |
